In-Office Dental Plan 

Which plan you receive depends on whether or not you have gum disease:

Plan A Cost: $385                                                         Plant B Cost: $585

Terms and Conditions: 

• All benefits must be used within 365 days. They do not “roll-over”.
• Enrollment fees must be paid in full and are non-refundable.
• Benefits cannot be transferred to other patients or family members.
• Partial refunds will not be issued for unused services.
• Benefits cannot be combined with other discounts or offers.
• 10% off does not apply to Orthodontics or merchandise such as electric toothbrushes.
• Terms and conditions of both plans are subject to change without notice

**If you do not show up to your appointment or cancel less than 24 hours before, you forfeit one of your included cleanings**

 Plan B Members:

• Please note the initial treatment for gum disease (“scaling”, a “deep cleaning”, or “periodontal surgery” is NOT included in the $595 fee, although you will receive 10% off of those procedure.
• Periodontal maintenance (the cleanings you receive 3-4 times a year after initial treatment) is included in Plan B.

 New Patients:  Enroll at your first appointment to receive $100 off your initial exam and x-rays.
